Changeroom and Athletic Lockers

Students may use this form to request a change room locker or, if eligible, an athletics locker for sports equipment storage.

There are also larger lockers for athletics in the hallway by the gyms. These lockers are limited and are reserved for school athletes and Academy students.

Locker numbers and combination information will be sent to students' school Outlook email accounts.

By requesting a locker, students agree to:

• Keep the locker and lock clean, in good working order, and free from graffiti.
• Keep the locker locked at all times and keep the combination secure.
• Return the locker in acceptable condition at the end of the school year.

Lockers and locks will be inspected by a staff member at the end of the school year. Any damage beyond normal wear and tear may result in the student assigned to the locker being responsible for repair or replacement costs.

Lockers remain the property of the school and may be searched at any time.
